On the board is working on hospitals following hte ten steps to breastfeeding from WHO or model policies from California, lactation accomodation, follow up visits and prenatal education, education in early years of school, medical school education and residencies, showing that California is unique and people breastfeed here!! Help us out, yell it out; what should we focus on? Her passion is about womens rights and has been. They have about 40 inspectors and 20 officers all over the state. They have been told to drop other complaints when a complaint comes in from a mother that she is not able to express her milk. The more they can educate and raise awareness to get the complaint to them; another citation about 3 weeks ago, and Angela is taking us through the website so we know how to make a complaint. Angela thanks Karen Farley and the CBC for letting her know what needs to be changed.&lt;br /&gt;&lt;br /&gt;Ethera lets us know this is a serious statute. It is a newer statute for the division, so awareness and education must start. Therefore staff know where the complaint should go. When one comes in, they will be there within a couple of days. Web now says Labor Commissioner so people know where to go. Then go to FAQ, and then click Lactation Accommodation...scroll down and there are links to different areas as well as explanations. Help get the word out. Citations are per offense, $100 each time. Several complaints have come in and employers must adhere. The most recent the employer has agreed to pay, without appeal. If a mother says she wants to express and there is retaliation, there is a complaint process for that. There are forms and handouts about this too. Employers should have these posted. It is great that there are new policies that can be posted, revised October 2009! Thank you Labor Commissioner.....&lt;br /&gt;&lt;br /&gt;Questions were asked about employers that say it hurts them? Angela says the burden of proof is with them to prove that.&lt;br /&gt;&lt;br /&gt;Angela says that mothers must be given a private area, not just a private room but a private time to express.&lt;br /&gt;&lt;br /&gt;Audience asks about time frame? It must be filed within 1 year from it happened. 6 months for retaliation.&lt;br /&gt;&lt;br /&gt;Panel member asks about time for pumping: Angela lets us know that every mother is different and each case may be different. It says reasonable within the law.&lt;br /&gt;&lt;br /&gt;Audience asks about language needs: web site is only in English but forms are in both. Angela addresses that this is an issue and they are working on the funds to begin this. At least the forms are in different languages.&lt;br /&gt;&lt;br /&gt;Audience asks: since so few complaints be stream lined? Angela and Karen Peters acknowledge that this change is great. Angela again thanks CWA and CBC for bringing this up.&lt;br /&gt;&lt;br /&gt;Audience member: how is implementation going with state employees? There are 3 areas you should know: &lt;br /&gt;&lt;span style="font-weight:bold;"&gt;NEW WIC FOOD PACKAGE&lt;/span&gt;: juice has been cut in half, more cultural choices, milk cheese and eggs cut in half to make room for whole grains and a cash value check for fruits and veggies! Formula amounts were the same before, did not matter how you were feeding. In teh second half of the year it was a lot of formula. Now it is based on the feeding choice and the amount that is actually needed. At 6 months is it cut in half and they get fruits and veggies as well as food and meats. No juice is gone until 1st birthday here. So new terminology was needed; fully breastfeeding mothers get twice the amount of infant foods at 6 months, mostly and some breastfeeding also defined. When they talked to families about this they get they will get more food if they breastfeed. If they want to breastfeed, there is no formula! We are helping the mother to build her supply, if she says she wants to get formula there is a process they go through to get it. Mothers can get some if they are breastfeeding a little but their food stops at 6 months. If they want formula they do a nutrition assessment, offer breastfeeding assistance, and work if there is a nutritional need with the smallest amount of formula given for one month. We want to keep them breastfeeding&lt;br /&gt;&lt;br /&gt;So what has happened? In LA we have seen at one clinic; the only number not changed is mothers who only want to formula feed. However those who were combo feeding went down and exclusive breastfeeding went up. This change happened before food package even rolled out, just with language change that staff were making. You can make a small policy change and see what a positive impact as Michelle shares. The education worked PRIOR to the food change....we know it works!&lt;br /&gt;&lt;br /&gt;&lt;span style="font-weight:bold;"&gt;Another change.&lt;/span&gt;..&lt;br /&gt;Every person on WIC payroll has to have 20 hours of BF training that must be done by Sept 2011 and many have started this.&lt;br /&gt;&lt;br /&gt;Michelle shares that we had 2 million dollars to do peer counseling programs per year, but a note came through that the USDA was going to increase this. On Friday we found out that 4 million would be given....then the email came and 6 million was told the state would get....now over the weekend 12.3 million was given!!!!!!! Congress is giving this money and we need results.&lt;br /&gt;&lt;br /&gt;Jackie Kemp now explains (in 5 minutes!) that she is a lone nurse at WIC  : )&lt;br /&gt;There are several peer counseling throughout the state in all settings. USDA has funded this in the past since 2004. They require we use the Loving Support Model and it was put together by Best Start. They gave training to all states, and there have been 15 programs funded in CA out of 82. All agencies have core services, but IBCLC's are availabel to refer to when out of their hands. Supervisor oversees as well. The most successful programs have done a community assessment and then geer towards that. Having an enhanced contact at certain intervals. Not just a phone call and not just one. Those that have on going training and support do better, have follow up meetings and talk about cases, refer cases quickly to an IBCLC who can work with them. Integrate within the WIC program so they are included into everything at WIC. The funding came from Title V funding. They worked in this area because there were areas within CA that maintain very poor breastfeeding rates. She shows a map broken into the Regional Perinatal Programs because they work on QI with laboring and delivering hospitals in these areas. The central valley and in LA and Orange County these rates were very poor compared to others. Also the key was difference in any and exclusive breastfeeding. We expect this gap to be small but it is not. Investing in CA's Future did have a recommendation that hospitals and clinics promtote exclusive BF for the first 6 months of life. There is a particular strategy that states that we maintain and disseminate model hospital policies and promote this. The model hospital policy recs were developed by the Regional Perinatal Program in Loma Linda and the toolkit was developed by Jeanette Panchula who was hired by the state to build this; many links and resources. see cdph.ca.gov/breastfeeding for this area. &lt;br /&gt;&lt;br /&gt;All information is on the CDPH website for hospitals. Some years ago some money was given to work with hospitals in these worse areas to develop the Birth and Beyond California Program. Developed from Loma Linda with Carol Melcher's project and it had 3 areas: must have a QI team where policies are being worked on, training in boht a 2 hour pre-meeting and then 16 hours with 20 staff members and then another 16 hours of 4 of those staff to be train the trainers, the last was that each hospital met as a group once per month to share successes and issues with each other. The state gave data and technical support and regionally IBCLC's to give training to hospitals as well as PACLAC who had a large pool of IBCLC's to assist with program.&lt;br /&gt;&lt;br /&gt;Hospitals had to apply and compete for program. It started with a self appraisal questionnaire, pre and post tests, self efficacy surveys, and we then gave the hospitals a data collection template. We could not mandate that but we assisted them to getting the data.&lt;br /&gt;&lt;br /&gt;MCH was hit with the wirst cuts and the program was cut, but LA is making a report and trying to keep it alive through that.&lt;br /&gt;&lt;br /&gt;Karen Peters explains that we thought the program would work because of the research. The CDC survey parallels the information we wanted and we hope to work more with that. Rosenburg tells us that what happens in the hospital stay makes a difference. At start we asked that CEO's as well as others be in the room, along the way we found that we needed the CEO and CNO were involved from the start the outcomes were better. We also knew that data over time was better, so we had the template to keep track. Most staff said that their patients were different. Anne Merewood shows us that all race and ethnic groups change when your policies were addressed. Murray showed that even at 8 weeks breastfeeding data was different when their were 5 practices shown; BF at one hour at birth, no pacifier, rooming in, exclusive feeding at hospital, and having a number to call. Also Oregon showed us that even banning the formula gift bag raised rates.&lt;br /&gt;&lt;br /&gt;We then tried to make environment better to help mother. So we need to keep moms and babies together; skin to skin. This is for the well being of both, and nurses work to keep this couplet stable. It has been an easy sell because everyone likes it. It increases our infants ability to feed. Moms, Dads, and babies love it. We then have a positive spiral of feeding babies with less problems. We encourage maternal confidence by doing all of this as well. We found that hospitals increased more policies and their feeding data changed as policies changed. There are more working on and have changed in 6 months than before. The chart she showed does not look at feeding but looks at model policies they are working on and doing. We cannot compare this to their data because we were not able to ask them about feeding.&lt;br /&gt;&lt;br /&gt;Panel member asks about data: Karen explains that we look at skin to skin, rooming in and exclusive breastfeeding.&lt;br /&gt;&lt;br /&gt;Panel member asks how many hospitals: Karen explains that there were 3 cycles with different numbers every time in all 3 regions.&lt;br /&gt;&lt;br /&gt;Karen goes onto explain that we sold skin to skin and infant and maternal competence. We found we needed to help and make infant safety their priority; safe in arms and monitored. It is a systems change program, not a breastfeeding program. The network meetings were critical. Nurse share how well it worked and areas they work on one at a time. Most hospitals only collect data on patient satisfaction. We want the QI people there because they know what they are doing. Data is essential, we encourage them to do so and look at sustaining and how they continue to support project. Some went baby friendly in the process because they needed help with sustaining. All hospitals did move on to train others at their hospitals. No one objected to skin to skin before bating the baby; nurses or docs. Patients did not need the skin to skin and visitors did not object to not seeing baby right at birth. The Golden Hour was respected...as it has come to be called.&lt;br /&gt;&lt;br /&gt;Joint Commission has accepted the exclusive breastfeeding as a standard as a quality measure as of 2009. Lets build on this and work on maternity care. Carina knows personally about policies and procedures in hospitals because her first was born and entered into a NICU, now has a new little boy that graced us with his presence earlier.&lt;br /&gt;Mike explains how we get some of our breastfeeding data and the changes that have been made to the newborn screening form; these have been significant changes and why this is important. Some trend lines will disappear because of changes and after 2 years that trend line will re-appear.&lt;br /&gt;&lt;br /&gt;Carina explains that the data comes from the NBS form, all newborns should get in hospital 24-48 hours of age. There have been recent revisions in 2007-2008. We have an example of the form that was used where there was not an area for NICU babies who got maybe TPN or fortifier; so there were many others as well as breastmilk. So changes were made and changes were made. Data on website is through 2007 and items such as the TPN has been taken out. There are county tables, state tables, ethnic categories, etc. See the cdph website for these details (short cut at calwic.org). However we needed to know whether a baby was given TPN. This was added back in as well as stating that a chart review should be done to ascertain the data; not just asking mother. Banked human milk is on there as well. This hit the field in late 2007 and 2008 which will be used for the data that is being done right now. Total number of records is lower in 2008 because some were not reported on the correct form; some reduction in combo feeding and increase in breast only as well as formula only. It is an understanding that there were trainings where hospitals were told to mark what baby was given up until test as opposed to the whole time baby was there; some may not know about feeding. 2008 data will be looked at the same, excluding TPN as well. If there was anything outside of the breast except for TPN and IV then they are other.&lt;br /&gt;&lt;br /&gt;Concern is being raised about data being contaminated because of verbal instruction that most were not doing a chart review. Carina clarified that there will be many notes with the data to clarify. You will be able to compare past year but 2008 please do not compare to past years. We are hoping that 2010 will be a new data trend. &lt;br /&gt;&lt;br /&gt;Can the NICU data be separated is raised by a panel member? There is an area to ask where heel stick occurred and they are looking at splitting data by where the information was gathered. Mike lets us know that we will have good data for a certain time but we are not able to know what hospitals got the information in June that only the prior feed was to be coded; not the chart review that is supposed to be done. Hospitals will need to look at their own data but 2008 will have to stand alone and cannot look at previous years. Overall the data tells us what has happened in a hospital but not comparing it to previous years. Especially now since the National Quality Forum is now looking at Maternity Care Measures and wants the NICU babies pulled out. This wants all babies pulled, not just ones who are still in NICU but even those who were in and are now out. &lt;br /&gt;&lt;br /&gt;Another panel member said that they were told to change the timing of the heelstick which they were told to do as close to discharge as could. This is an issue because of maternal complications; also if some are doing at 24 hours as opposed to 36 hours. &lt;br /&gt;&lt;br /&gt;Another asked what the minimum time is? Carina clarifies that it says 12 hours is minimal. 24-28 hours is most frequent.&lt;br /&gt;Another panel member asked how new forms go out and how old ones are gone? for 2008 we know that 92% were done on one form and the others had begun with the newer one or even an older one. So they have to weed those out.&lt;br /&gt;&lt;br /&gt;Another panel member explains that there are many areas within the state that operates to get this data collected and distributed. Lets remember why we do this screen however. It is not for the feeding data, and many feel that the disease screening is being complicated by the feeding data being asked. So how can we meet both? So for 18 months our data is not the best, but will be better.&lt;br /&gt;Carina explains that the compromise: TPN must be there because it interferes with the screening, so there is simply a spot that asks yes or no if they receive.&lt;br /&gt;&lt;br /&gt;Current we will look at on the new forms exclusive BF, any BF, and denominator for both which will exclude those that are marked TPN/Hyperali or other not known. She showed us our rates and gaps of exclusive and any, but we will not see changes on this with data issues. Carina also showed us the slide that looked at ethnic groups of any and exclusive as well as broken into maternal county of residence. Carina explains that the data has been used by many and many will need to be told not to use the data for awhile. Cal Compare and CWA both use this data. We want this to be used in the future as well.&lt;br /&gt;&lt;br /&gt;Another panel member asks if a mother refuses the genetic test what can be done? They come to the state as blank and they are left that way. They then lump into unknown and missing.&lt;br /&gt;&lt;br /&gt;Audience member says that Federal guidelines are asking for birth certificate data; where are we with this? Mike lets us know that needs a legislative approval to happen. We have not began work on this.&lt;br /&gt;&lt;br /&gt;Audience member asks about getting data from childrens hospitals. One year ago when the CDC Grant was funded for Obesity she left WIC and is now with the Cancer Control Branch (Nutrition and Physical Activity trying to change name).&lt;br /&gt;First off, thank you for all the help that you are going to give over the next 2 days. Laurie is going to go into detail so forgive the post if it says I and should be Laurie!&lt;br /&gt;&lt;br /&gt;The first work group has already been started for you and your expertise is needed in all areas! Lets work from previous reports to add, change, and make better. Some may ask why we are putting together another plan with such a great report? Because this is an Obesity plan and breastfeeding is only part of that. WIC supports BF and so does the MCH branch...thank you Suzanne Haydu! The bad news per Laurie is that she is the only one with a background in BF promotion in her area so she needs help!!!! BF is not really on the table so we need to find what is most important to put into this new plan....on Obesity. Some counties have applied for funding to help in some of these areas and the state is working with the smaller counties (Tulare and Sonoma).&lt;br /&gt;&lt;br /&gt;So what strategies and activities are most important to you? We are going to give you 15 strategies to look at; do you think something is missing? We can put it back in or add it!&lt;br /&gt;&lt;br /&gt;History lesson on Obesity Prevention: 2005 the Governor had a vision for a healthy california which put Obesity on the radar and made it a priority. Then a Get Healthy workgroup came together and made the CA Obesity Prevention Plan in 2006. There was originally a large pot of money to help, but we could not get all that. Then the Dept of PH split from the DHS and this current plan was applied for! So we are actually enhancing out plan and making it more detailed. $730,398 per year for 5 years. Laurie is one of five connecting everyone to create and implement the creation plan through policy and environmental changes. &lt;br /&gt;&lt;br /&gt;Goals: central point of contact within state government, statewide media campaign frames healthy eating and active living as CA living, support local assitance grants and multi-sectoral policy strategies, and create and implement a statewide tracking and evaluation plan. We do not have money now; however if we had money what we would like to see done? What would help? This is why we are making this plan! &lt;br /&gt;&lt;br /&gt;CDC told us that breastfeeding had to be in this plan which is very important! There are 6 target areas: decrease high energy dense foods, drink less sugar sweetened beverages, decrease television viewing time, and increase physical activity, increase fruit and vegetable consumption, and increase breastfeeding promotion to raise rates and duration.&lt;br /&gt;&lt;br /&gt;So what is the evidence for intervention for obesity? There were several documents reviewed such as the IOM local government actions from Sept 2009, CDC community strategies July 2009, Robert Wood Johnson Tool Kit....and what is out there? Lots of counties have plans, lots of local areas have plans...even CalTrans has a plan!&lt;br /&gt;&lt;br /&gt;How does this tie into breastfeeding? There are only 3 in the original plan that apply : health care and insurers and providers support breastfeeding through supporting mothers in exclusive and prolonged breastfeeding and providing continuing education for providers, as well as employers posting and enforcing policies. We have looked at these and made more defined:&lt;br /&gt;  &lt;span style="font-weight:bold;"&gt;Hospital Policies&lt;br /&gt;  Peer Counseling&lt;br /&gt;  Lactation Accomodation&lt;/span&gt;&lt;br /&gt;&lt;br /&gt;So we convened a provisional committee to get this rolling....there were 13 strategies to review and asked to prioritize to top 8. Put into workgroups and drafted work in all areas. We will be looking at what we wrote that day! Modify as you wish. Also 2 new stratgies were added into state and local government areas; mother to mother support and breastfeeding data collection strategy will be reviewed by the Evaluation Advisory Committee at a later date.&lt;br /&gt;&lt;br /&gt;We will be evaluating the progress on this plan. We want to see that people change their behavior, policies change, environment changes, and overall OBESITY changes! So this is a public plan. There were listening sessions held at the network conference, childhood obesity conference, school wellness conference and then community forums in Orange County, Redding, Central Valley, and the Bay Area.&lt;br /&gt;&lt;br /&gt;Now we need you to choose a workgroup (there are 5) to identify recommended activities, determine whether there are missing strategies, and prioritize strategies for the CBR to tackle. So we cannot all work together in every group; please choose a group and look at the workgroups. There are the following for workgroups with 5 panel members in each committee and then the audience will work into groups as well:&lt;br /&gt;  &lt;span style="font-weight:bold;"&gt;State and Local Government&lt;br /&gt;  Healthcare Insurers/Providers&lt;br /&gt;  All Employers/Families/Communities/Schools&lt;br /&gt;  Food &amp; Beverage Industry&lt;br /&gt;  Entertainment &amp; Professional Sports&lt;/span&gt;&lt;br /&gt;&lt;br /&gt;Our timeline: draft document to management by the end of this month, we are supposed to publish the plan in the spring. Steps that must be taken within the Dept of PH, made from former BPAC through 2007.&lt;br /&gt;Lets look at these in no particular order: &lt;br /&gt;  &lt;span style="font-weight:bold;"&gt;Professional Education&lt;/span&gt; (training, continuing education, adopt legislation requiring  education)&lt;br /&gt;  &lt;span style="font-weight:bold;"&gt;Health Care Systems&lt;/span&gt; (all CA women have education to BF, ensure all hospitals and clinics support exclusive BF for first 6 months, assessment and intervention in the first weeks postpartum, program should support mothers to BF, insurance plans should find way to reimburse BF services)&lt;br /&gt;  &lt;span style="font-weight:bold;"&gt;Public Education&lt;/span&gt; (incorporate infant feeding into all levels of education, promote positive images of BF, eliminate bottles as icon representing infants, consumer guide on hospitals looking at rates as well as for doctors offices, support BF promotion through local coalitions)&lt;br /&gt;  &lt;span style="font-weight:bold;"&gt;Mother to mother &lt;/span&gt;(culturally sensitive and linguistically appropriate education, effective models of this support, make sure these supporters receive training, availability of programs and knowledge of, establish and maintain communication of these programs, provide recognition for outstanding programs)&lt;br /&gt;  &lt;span style="font-weight:bold;"&gt;Workplace&lt;/span&gt; (improve legislation, encourage BF friendly environments, encourage all business and educational sites to provide lactation accommodation, license info for child care providers)&lt;br /&gt;  &lt;span style="font-weight:bold;"&gt;Assessment and Research&lt;/span&gt; (keep up wiht this info and look at within when changes occur, ways to overcome barriers we hear about, collect data related to excellence indicators at local and regional levels, support research and healthcare system in deterring women from exclusive breastfeeding, develop tools for professional and para-professional who are at risk and need help, evaluate cost effectiveness of BF, develop mechanisms for all of these)&lt;br /&gt;&lt;br /&gt;So where do we go says Robbie?? We come here! We can coordinate at all levels of what we are doing. Lets give oversight. We are such a diverse place....lets address this in all areas of breastfeeding...and of course the big area we all know...$$$$$!!! Think about can we even do what we are thinking? Do we need to change strategies. &lt;br /&gt;We will increase our breastfeeding rates. We picked people from different areas of the state as well as backgrounds.&lt;br /&gt;So what is the mission? To provide policy recommendations and strategies to increase breastfeeding rates and duration. We want BF to be the exclusive food for 6 months and then adding solids throughout one year an beyond; on par with the AAP statement as well as ACOG.&lt;br /&gt;Steering Committee made by-laws; members given copies in their packets. CWA represents 82 agencies throughout CA and the CBC represents the 41 local coalition throughout the state..we even have a new logo for the CBR.&lt;br /&gt;CWA has been partnering with other agencies that are working on policy changes; gained allies such as the Labor Commissioner and national coalitions such as the USBC. Laurie from CWA goes to DC next week!&lt;br /&gt;The CBC has a diverse membership of local and state coalitions that has so many great backgrounds rolled into one with grassroots support with legislation! Thank you followers! We all worked so hard on this as well as employer awards, walks, and other outreach efforts....you all work so hard for this! We work hand in hand with the MCH Department and the USBC; we represent the Western Region for this through Karen Peters.&lt;br /&gt;(we have a sheet with all these acronyms remember that!) She gave a great presentation that was wrote by Laurie Pennings, MS, RD, IBCLC&lt;br /&gt;Wow so many acronyms! We even have a handout today to go through that.&lt;br /&gt;BPAC was the original idea in 1993; Breastfeeding Promotion Advisory Committee and had a goal to make recommendations and strategies to form a blue print for breastfeeding. Much like today had a diverse group and looked at 6 areas; professional education, health care system, public education, mother to mother support, workplace, adn research.&lt;br /&gt;3 years later the first report published and was award winning because it was so well done! Looked at rates, recommendation, and all based on scientific literature.&lt;br /&gt;1997 CA put into the civil code that it was allowed wherever you were allowed to be. At this same time many divisions came together and met to discuss breastfeeding. Medi-cal plans were urged to support pumps and services.&lt;br /&gt;1999 letter went to hospitals to attempt to adopt Model Policies and ask schools to adopt this into their curriculum. CMS developed protocols too! Wow 1999 was big! To top it off hospitals responded to surveys at 100% and UC Davis began to looks at policies and practices.&lt;br /&gt;2000 Best start social marketing campaign began and another law for jury duty to postpone for one year!&lt;br /&gt;2001 Lac Law passed that allows moms to pump (time) and place (not a bathroom). WIC sent letter to businesses referring to resources that was co-wrote from Labor Commissioner.&lt;br /&gt;2004 WIC starts peer counseling funds and trained 50 IBCLC's&lt;br /&gt;2005 40 local coalitions formed to make the CBC! Also another bill passed that said that the state recommend training for hospitals that are within the lowest quartiles and funds peer counselors if there are finds, and stream lined medi-cal&lt;br /&gt;2006 law passed to exempt hospitals to have a tissue bank license for mothers who are pumping milk for their babies&lt;br /&gt;On-going...well we will hear a lot about this throughout the day! Business Case for BF, Hospital Policy training, Employer Awards and Walks, WIC food package changes (less formula to all mothers)&lt;br /&gt;Now....BPAC has always met and got a great relationship and audience present. So much has been done from breast pumps to barriers...more reports and the last in 2007 "BF: Investing in CA's Future"&lt;br /&gt;All thigns change and the DHS splits! so BPAC lost home  : (&lt;br /&gt;With all good time comes great things...now with the Dept of public Health we got a HUGE Obesity grant that looks at evidence based policy change....and BF must be included in those target strategies!!!! HOORAY!!!&lt;br /&gt;So...in April formed the committee that we built from to meet here today.
